The versatile fullback had featured in 17 PL games before his winter exit, that number meant he was eligible for a medal.
Cancelo has made 15 Bundesliga appearances for Bayern Munich and steadily grew in influence as Bayern fought hard to win the title on the final day.
The Portuguese is not the only player to achieve such a grand feat however.
Daniel Amartey and David Beckham round-up exclusive trio
Daniel Amartey also achieved this feat by winning the Danish Superliga with FC Copenhagen and the Premier League with Leicester City in the 2015-16 season.
The midfielder played 15 times for the Danish side before his January move to the Foxes, where he made five appearances in the league to secure his medal and his own part of the club's underdog story.
David Beckham also won two titles in the same season as he won the MLS Cup with Los Angeles Galaxy and the Ligue 1 title with PSG during his stint in the 2012/13 season.
